Slovianka, a restaurant, cafe or canteen at 83 Far Gosford Street, Coventry was given the score after assessment on June 30, the Food Standards Agency's website shows.
It means that of Coventry's 515 restaurants, cafes and canteens with ratings, 392 (76%) have ratings of five and none have zero ratings.
