Paradise Pizza, a takeaway at 217 Stoney Stanton Road, Coventry was given the score after assessment on May 18, the Food Standards Agency's website shows.
It means that of Coventry's 313 takeaways with ratings, 200 (64%) have ratings of five and none have zero ratings.
