Peri Peri Original, a takeaway at 6-8 Carter Road, Coventry was given the score after assessment on August 23, the Food Standards Agency's website shows.
It means that of Coventry's 320 takeaways with ratings, 201 (63%) have ratings of five and none have zero ratings.
